Enrollment Applications

2015-2016 Admissions Application:

The application posted above is to add your child to the upcoming 2015-16 Admissions Lottery. The enrollment application period runs from January 2 until April 1, 2015 with the lottery to be held April 7, 2015. Simply print out the application, fill it out and make sure we receive it by 4pm April 1, 2015. One application is required for each child.

All applications must be mailed to 36-24 12th St. Long Island City, NY, 11106 to ensure proper entry into the lottery.

Please note that new applications are required for each school year. If you previously applied and are currently on the waiting list, that list will expire on June 30, 2015.
